This is a mandatory form.*
If you are serving documents by post or electronic communication you must include this form with the documents you are sending because unless the person served signs it and returns it to you, you will be unable to prove service. You must complete Items 1 and 2 on this form before you send it to the person being served.
If you are serving documents by hand you may include this form with the documents you are serving because if the person served signs it, it can be used to prove service. You must complete Items 1 and 2 on this form before you serve it.
If you are the person who has been served with an application or other documents, you may sign this form to admit that you have received the documents.

What you file with this form
If you are the person who has to prove service of documents, you must attach this to an Affidavit of Service. The only exception to this is if a lawyer has signed the Acknowledgment on behalf of the person served, it can be filed without being attached to an affidavit.
If you are the person who has been served with documents,you do not file the acknowledgment. You return it to the person who had the documents served on you.
