Affidavit – Non-Filing of Family Dispute Resolution Certificate




PLEASE READ BEFORE COMPLETING THIS AFFIDAVIT

If you are applying for interim orders, that is, orders to cover your case until a final hearing can take place, then you do not need to complete this affidavit.  You will have to complete an Application in a Case and an affidavit in support. You should ensure that that affidavit also sets out those matters which you rely upon to file your applications without filing a certificate from a family dispute resolution practitioner.

You do not need to complete this affidavit if there is a consent order or if you are applying for orders in response to an application.
