Affidavit of proof of signature



This form has been prepared by the Court for use when a person is required to file an affidavit in the Family Court stating that they recognise the signature on an Acknowledgment of Service.

When do you file it
If you have served a document on a person and you need to rely on an Acknowledgment of Service to prove service.

What do you file with it
It must have the signed Acknowledgment of Service attached to it.

Number of copies of the affidavit for filing, service and yourself
The completed and signed original of the affidavit filed at the Court. Before you file it you must also make sufficient copies to have one for each person to be served and a copy for your own records.

Nothing

In completing a Court form, you must
Complete the form by typing (eg. on a computer or typewriter).
