Case information



The Court may order that a party exchange with each other party and lodge with the Court this form providing case information prior to the first day before a Judge.

It includes:

  • A minute of the orders sought by you
  • A list of the witnesses you propose and a short outline of the nature of the evidence of each witness
  • A brief chronology of relevant events.


Number of copies of the form for filing, service and yourself
The completed and signed original of the form is lodged with the Registrar or Judge’s Associate at the Court registry. Before you lodge it you must also make sufficient copies to have one for each other party and a copy for your own records.

In completing a Court form, you must*
 Complete the form by typing (eg. on a computer or typewriter) or hand printed in ink.
