Affidavit Proving Signature (Divorce)




This form is used by a person filing a sole application for divorce, to state that they recognise the signature of their spouse on the Acknowledgment of Service (Divorce).

The signed Acknowledgement of Service (Divorce) must be attached to the Affidavit Proving Signature (Divorce).
