Application for Divorce Form

The Application for Divorce is available in different formats so you or your lawyer, if you have one, can choose the most appropriate one for you.

Note: The PDF version does not have a "save as" function. You should enter your details and then print the document.

Attachment for additional children
The Application for Divorce includes details of four children. If you have more than four children, use the Attachment for additional children form which can be found on the divorce forms and kits page to provide details of additional children. Once complete, fasten this attachment to the back of the Application for Divorce.

eFiling of divorce applications now available
Applications for Divorce (and certain accompanying documents) can now be electronically filed through the Commonwealth Courts Portal (www.comcourts.gov.au). For more information see the User Guide to eFiling Divorce Applications in Family Law, available at www.familylawcourts.gov.au
