Credit card payment form
This form must be used when authorising the Courts to make a payment from your credit card. The form must accompany any mailed applications or requests that require payment. Please do not fax this form unless requested to do so by the Court.
The amount that you authorise to be taken from your credit card must be in Australian dollars and must be the same as the fee that is payable to the Court. If you are uncertain of the correct amount, please call 1300 352 000 or email enquiries@familylawcourts.gov.au
You do not need to send this form if you send a money order or cheque as payment.
