Document request form
I need to prove we are divorced - how do I do that?
Your Certificate of Divorce is your official proof of divorce.
How do I get a replacement certificate of divorce
If you need a replacement Certificate of Divorce, you will need to complete a ‘Document request form'. Before completing the form, contact the Family Law Courts on 1300 352 000 or ask at your nearest registry so staff can tell you what details you need for the form and what fee you will have to pay.
You can file the completed form at any registry by post or in person. If you are unable to file the form yourself, you may authorise another person to do it on your behalf.
More information can be found in the Family Law Courts form 'Document request form' under the Forms section of this website.
